This repository contains the project materials and presentation developed during Code.Hack, Island Health’s first healthcare-focused hackathon, held at Royal Jubilee Hospital in Victoria, British Columbia, from March 8–10, 2019.
The event brought together interdisciplinary teams to rapidly develop innovative solutions addressing real-world healthcare challenges within an intensive 24-hour collaboration environment.
Code.Hack was organized by Island Health to foster innovation in healthcare through collaboration among clinicians, patients, data scientists, consultants, and technology professionals.

Our team identified a major challenge faced by chronic patients in storing, managing, and transferring their medical records across different healthcare providers and systems.
Many patients are required to repeatedly transport physical records, communicate medical histories manually, or navigate fragmented healthcare information systems.
To address this issue, we proposed an innovative cloud-based solution designed to provide:
- Transferable medical records
- Accessible personal health archives
- Improved continuity of care
- Reduced administrative burden for patients
- More efficient communication between care providers
The proposed solution focused on empowering patients through secure and portable digital health information management.
The TAPA Champions team included a multidisciplinary group of participants representing diverse healthcare perspectives:
- Nurse (Code Blue)
- Healthcare Manager (Code Blue)
- Patient Representative (Code Red)
- Industry Consultants (Code Yellow)
- Data Scientist (Code Yellow)
This interdisciplinary collaboration enabled the team to combine clinical, operational, patient-centered, and analytical perspectives into the proposed solution.
