Add design.md with Figma source of truth, Bootstrap 5.3.3 DS identity, desktop/mobile coverage status, usage hierarchy, Bootstrap customizations, and accepted overrides section. Remove design-related details from CONTEXT.md (Bootstrap description, style separation) to avoid redundancy with design.md.

I think we should rename the file to DESIGN.md (uppercase) and update the reference to it in CONTEXT.md. I don't think this file adds any real value (except the figma section) to the project at the moment, but I'm not against introducing it into the repo and reworking it later if needed.
