FFE forms step 4 - Add account data

This PR offers some of the missing features of #1678:

• Add contact information (phone/email) and FFE data (licence/arbiter level) to accounts
• Update the account modal (@pascalaubry)
• Add the information to forms (@pascalaubry)

TODO for next PRs:

• Replace multi-lines inputs by textareas (@timothyarmes ?)
• Add the possibility of marking the accounts with the ORGANISER RoleType (as we already do with CHIEF_ARBITER and DEPUTY_CHIEF_ARBITER)
