Optimize `IgnoreStateManager.loadAll` to load ignored files from multiple repositories in parallel using `Promise.all` instead of sequentially. This significantly reduces the total loading time when multiple repositories are open. Benchmark results (20 repos, 100ms latency each): - Sequential: ~2000ms - Parallel: ~100ms
