fix: tolerate headset input startup route churn

Summary:

• Treat immediate AVAudioEngine configuration notifications during headset startup as route setup churn only when the engine is still running.
• Keep AppState/hotkey recording active for that healthy startup notification instead of forcing recovery.
• Preserve forced stop/reset recovery for later device changes, or for startup changes where the engine actually stops.

Tests:

• swift test
• GitHub Actions CI: debug build, tests, release build, and app bundle verification passed on 5d84c60.

Test build:

• VocaMac-0.7.0-pr.175+5d84c60-arm64.dmg
• Signed with Developer ID and notarized by Apple.
