This guide covers locations for progression and other items that I noted down as potentially tricky while re-playing the game.
It does not cover items that you can unambiguously locate with in-game methods.
You can click on images to open full-size versions of them.
The guide does not contain story/enemy/boss spoilers, but it does show different places in the game world.
With that out of the way,
Here's a thing to know about the game: it's pretty doable to get all the achievements, but the difficulty-based achievements are granted based on what the lowest difficulty was on the save file.
So, for example, if you've started the game on Explorer difficulty and switched to Advanced later on, you will not get the "The Epic Journey Ends" achievements - better start a new save file if you want to go for all of these.
Aiming mode setting has no effect on achievements - you can use what you'd like with this one. I think auto-aim is neat when playing on a controller as it lets you move some movement inputs to face buttons instead.
I'm making these a separate section as you can unlock a bunch of them in whatever order you like.

The introductory dungeon is pretty normal, there are just a couple not-so-hidden items here:

First, you need to go south to the woods, do a little hook to the east, and find this building:

It will lead you to the area west of where you have entered, and you'll be able to access the dungeon's whereabouts by going north.
But there's another interesting thing in the area: this building!

And now back to that dungeon.
To get to the objective, you'll need to get in this hole:

Things are straightforward from thereon.
While you'll be passing by the south-west corner of the map, check behind the suspicious statue in a chapel-looking building for a useful item.
The dungeon has just a couple additional items, a scarab (north-east, hidden wall) and a heart fragment (north-west).

The dungeon's upgrade, however, will allow you to access a handful new places across the map.
First you go to the eastern area:

One is open, other is opened by pressing a button under a pile of rubble.

The dungeon's alright. If you're playing on Advanced difficulty, remember to use the abilities that you have hopefully acquired from optional areas.
There's a checkpoint and an option to exit the dungeon just before the boss fight, so you can try this even if you feel like you could use more upgrades. When you re-enter later, you won't have to re-do the whole thing.
So you've beat the game, but what's a game without an optional bonus dungeon?
You'll need the item that you can get after completing all the races to get in there.
And after you're done with that, there's another bonus dungeon that you can try!
This one has different challenges with revised boss fights in it.
After you're done messing around with these and are all-around satisfied, in the same dungeon you will find an option to formally finish the game, obliterate your save file out of existence, and get the (hopefully the last remaining) "Without looking back" achievement.
