Initial xAPI 2.0 Release

Initial release to support the IEEE 9274.1.1 release -- also referred to as xAPI 2.0.

The 1.0.3 functionality has not changed.

The 2.0.0 tests incorporated new request handlers which are in the process of being integrated with the verbose error output and HTTP traces as the original, but the older syntax was prohibitively cumbersome when trying to meet the original timeline. These 2.0 logs and repository links will be updated in the future to assist developers.

Fixes issues: 20, 22, 23, 24, 25.

Update version to 1.2.2.
