ci: Remove Phase D mentions from ai-review-command workflow

Removes "Phase D" terminology from the AI PR Review workflow start message. This is part of a broader cleanup to remove legacy phase naming from the AI review system.

How

Simple text replacement in the workflow's start-message:

• **AI PR Review (Phase D)** → **AI PR Review**
• Phase D adds MCP-based validation verification. → with MCP-based validation verification.

1. .github/workflows/ai-review-command.yml - only file changed, just the start-message text

User Impact

Users will see cleaner messaging when /ai-review is triggered on PRs. No functional changes.
