fix(qoder): prevent cold-start trace replay

Summary

• persist per-transcript Qoder and QoderWork hook cursors outside the deploy-overwritten hooks directory
• classify cursor recovery and emit only the latest logical turn during bootstrap while preserving normal incremental turns
• add batch-scoped Trace Input replay protection shared by Qoder, QoderWork, and Qoder CN
• update diagnostics and add processor, input, and integration regressions

Validation

• 72 targeted Qoder/QoderWork/Qoder CN tests passed on latest origin/main
• npm run typecheck passed with the locked beta.9 dependency
• manual redeploy verification: three old QoderWork sessions skipped 5, 82, and 222 historical turns and each produced exactly one trace
