Adds a new customisation option in org-appear. By customising org-appear-trigger, it is possible to control when Org elements are toggled.

There are currently 3 trigger options:

• always :: this option corresponds to the standard behaviour of org-appear before this release. Elements are always toggled. This is the only mode of operation that respects the org-appear-delay setting.
• on-change :: elements are toggled only when the buffer is edited or when the user clicks an element with a mouse.
• manual :: elements are toggled only after org-appear-manual-start has been called. org-appear-manual-stop is used to stop toggling.
