Refactor mutation inference implementation and improve docs

This PR does the following:

1. Refactors the way mutations are configured
2. Implements the SSSOM generalization rules https://mapping-commons.github.io/sssom/chaining-rules/#generalisation-rules
3. Adds more docs on the README
