Update windows VC++ nmake makefiles, add saslPASSDSS. Now there are 14 plugins in 'plugins/NTMakefile'.
Update plugins/makeinit.sh to generate all 16 '*_init.c' files at once, using sh makeinit.sh.
Update plugins/passdss.c add #include <stdint.h> to make it compiled in vc-win32.

Tested with vs2019-x64, db-5.3.28, sqlite-3380500 and openssl-1.1.1o.
Tested with vs2019-x86, db-5.3.28, sqlite-3380500 and openssl-1.0.2u.

There are 14 plugins in 'plugins/NTMakefile', 12 of them were built success.
LDAP and GSSAPI have not been tested.
Plugins of kerberos4.c and gs2.c are not in 'plugins/NTMakefile'.

Some plugins are going to be deleted, such as kerberos4. May want to check the open tickets on that as well.

May be better to use CMAKE if that support is added, see #842

Irrelevant. We don't have the time or resources to support redundant possibilities.
