Enhancements

• Allow for manually configuring the Nuki bridge(s) en lieu of automated discovery, see #153.
  Note that you need to specify the bridge ID in addition to the hostname of each bridge. This is the id as returned by nb discover or the serverId as returned by nb info. I'm not sure where to find it in the Nuki app; due to the broken discovery server, it currently doesn't find my bridge.
  Also note that each bridge needs to have a static (statically assigned) IP address, as, without discovery, Homebridge NB is not able to detect the new IP address when it changes.
