Review agent outage: opus alias resolves to policy-denied claude-opus-5 on Vertex AI #1117

What happened

Since approximately Aug 27 20:56 UTC, every review agent run on redhat-developer/rhdh-plugin-export-overlays has failed immediately with Error code policy_denied when attempting to use claude-opus-5. The issue was confirmed across at least 12 runs spanning 4+ days:

The harness config at the pinned agents repo commit (4bbe4f50) specifies model: opus and image: ghcr.io/fullsend-ai/fullsend-code:latest. The :latest Docker tag was updated to include Claude Code v2.1.243, which maps opus to claude-opus-5 instead of claude-opus-4-6. The Vertex AI project returns policy_denied for claude-opus-5, causing zero tokens consumed and no review output on any iteration.

What could go better

Two compounding issues caused this extended outage:

  1. No model availability validation at launch. The fullsend runtime launches the agent, which immediately hits policy_denied and exits with code 1 after consuming zero tokens. There is no pre-flight check that the resolved model is accessible on the configured Vertex AI project. A pre-check before sandbox creation (which takes ~45s) would fail fast with an actionable error message naming the model and the project.

  2. No alerting on sustained agent failures. The review agent has been failing on every single PR for 4+ days without any automated alert or escalation. The failure is silent — the status comment shows 'Terminated/Failure' but no issue is auto-filed when a role enters a sustained failure state.

Confidence is high on the diagnosis — the logs are unambiguous (Error code policy_denied on claude-opus-5 with zero tokens), and the pattern is consistent across 12+ runs. The agents repo HEAD already moved the image to a SHA-pinned reference, but this repo's overlay pins to the older commit where :latest was used, and even a SHA-pinned image containing Claude Code v2.1.243+ would have the same issue since the model mapping lives in the Claude Code binary.

Proposed change

Immediate fix: enable claude-opus-5 on the Vertex AI project used by the fullsend fleet. This unblocks all repos using model: opus in their harness configs.

Preventive fix: add a model availability pre-check in the fullsend CLI's run command (before sandbox creation). After resolving the model alias to a concrete model ID, issue a lightweight API probe (e.g., a minimal completion request or model info endpoint) against the configured Vertex AI project. If the probe returns policy_denied or model_not_found, fail immediately with an error message like: Model 'claude-opus-5' (resolved from alias 'opus') is not available on Vertex AI project <project_id>. Enable the model in your GCP console or pin a specific model ID in the harness config.

This pre-check would catch the issue in seconds instead of letting the agent loop through 2 iterations of sandbox creation, bootstrap, and immediate failure (~2 minutes of wasted CI time per run).

Validation criteria

After the Vertex AI project is updated: the next 5 review agent runs on redhat-developer/rhdh-plugin-export-overlays should complete without policy_denied errors. Review status comments should show completed reviews with non-zero token usage and cost.

After the pre-check is implemented: trigger a review with an intentionally misconfigured model (e.g., a model not enabled on the project). The run should fail within 5 seconds of the fullsend run invocation with an actionable error message, without creating a sandbox.

The four overlay-repo runs named in this issue do not support the “opus alias resolved to claude-opus-5 and Vertex denied the model” diagnosis. The failures never reached Vertex.

Run When Result Transcript model Sandbox
    33108540066 Aug 27 19:27 success claude-opus-4-6 /usr/local/bin/claude allowed to STS and aiplatform.googleapis.com; POSTs to .../models/claude-opus-4-6:streamRawPredict
    33115696159 Aug 27 20:55 failure <synthetic> 39 denials: claude.exe → sts.googleapis.com
33384937403 Aug 31 11:00 failure <synthetic> same STS binary denials
33388714898 Aug 31 11:48 failure <synthetic> same STS binary denials

Last success (33108540066): review ran 4m 16s on claude-opus-4-6, 108 messages, ~51k output tokens, 63 tool uses. Binary on the wire: /usr/local/bin/claude. Vertex policy hits: 42.

Every later run: two iterations, ~1.7s, 0 tokens:

    API Error: Error code policy_denied

OPA reason is the executable path, not the model ID:

    /usr/lib/node_modules/@anthropic-ai/claude-code/bin/claude.exe -> sts.googleapis.com:443
reason: binary '.../claude.exe' not allowed in policy 'vertex_ai'

No aiplatform.googleapis.com traffic. No HTTP to claude-opus-5. The agent never authenticated, so Vertex never evaluated a model.

Claude Code 2.1+ ships a Bun compile named claude.exe even on Linux. **/claude does not match claude.exe, so STS is blocked and the transcript surfaces policy_denied. Enabling claude-opus-5 on the Vertex project would not have unblocked these runs.

The working fix is the binary allowlist. rhdh-parasol/rhdh-plugin-export-overlays#13 (fix(fullsend): allow Claude Code Vertex authentication, merged 2026-09-01) added path: "**/claude.exe" next to **/claude in the vertex_ai policy (review, plus code and e2e-triage), and pointed Review at the repo-owned image and rhdh/policies/review.yaml. That follows redhat-developer/rhdh-agentic#124.

    Root cause correction

The retro-agent diagnosis (opus alias → claude-opus-5 → Vertex policy_denied) is incorrect. As confirmed in the follow-up analysis, the failure occurs at the OPA sandbox binary-allowlist level, not at Vertex AI model authorization.

Claude Code 2.1+ ships as a Bun-compiled binary named claude.exe (even on Linux). The profiles/fullsend-vertex-ai.yaml binary allowlist permits **/claude but not **/claude.exe, so OPA blocks the binary from reaching sts.googleapis.com:443. Authentication never occurs — no traffic reaches aiplatform.googleapis.com at all.

    Fix

Add "**/claude.exe" to the binaries list in profiles/fullsend-vertex-ai.yaml:

    binaries:
      - "**/claude"
      - "**/claude.exe"
      - "**/pi"
      - "**/node"

A downstream fix has already been merged for one affected repo, but updating the upstream profile here protects all other repos using the default profile.

    Proposed test

    # CI assertion: profiles/fullsend-vertex-ai.yaml binaries list
    # must include both claude and claude.exe globs
    assert:
      - "**/claude" in profile.binaries
      - "**/claude.exe" in profile.binaries

Labels: Binary allowlist bug affecting the review agent, filed by the retro agent — matches bug and agent-role labels per repo conventions.
