Add ability to refine meshes in output (which use a basis order of 0 or 1) to better capture higher order discretization. This is primary used for things like capturing higher order discretizations on the fault and ground surface.
Updated examples/strikeslip-2d to demonstrate refinement of output and use a more realistic slip distribution.
