Adding Polkadot chain and Hydration AMM connector.

Some useful resources for testing:

• We recommend using Talisman Wallet and have funds in the Hydration parachain, which can be checked here (especially in HDX, DOT, USDT, and USDC (prefer Polkadot, not the Wormhole one, or edit the src/template/lists/hydration.json) tokens)
• To transfer funds from another chain to Hydration parachain their cross exchange functionality can be used
