fix(ai): keep panel state per tab and approve with Ctrl Enter - #1723

Remember AI overlay open/closed per workspace tab, toggle history with the existing shortcut, approve a single pending command with Ctrl/Cmd Enter, and keep xterm focused on mouse leave.
