Refine VOD playback controls and next episode skip

Summary

• keep VOD OK/back behavior predictable by showing controls on start and on OK pause/resume without affecting live playback
• rework next episode timing so the setting is the skip point from the end and a fixed 15-second countdown leads into that jump
• keep the next-episode overlay passive so it does not steal OK or left/right remote input, and allow Back to dismiss it for the current episode
• bump app version to 3.4.1
