Fail the upgrader when a tracked Addon enters Failed instead of treating it as settled. During dbfix, patch cluster-version-reader with get/list/watch access to apps.kubeblocks.io/componentversions when the dbprovider frontend RBAC exists, and skip cleanly when it does not.
