What does this PR do?

Stubs the community health docs to point to the main llm-d/llm-d repo

Why is this change needed?

To ensure we don't have to change community health docs across many repos, let's standardize on using the main llm-d repo for the source of truth.

@clubanderson What do you think about having a stub of the community health files back to the main llm-d repo? My fear is that we'll have lots of copies of docs to update if we need to update them in the future. Mainly having a goal of a single source of truth.
