docs(agent-feedback): migrate to one item per file

Moves the agent-feedback backlog off the five category files (bugs.md, cleanup.md, dx.md, perf.md, unclear.md) and onto agent-feedback/items/, one file per finding with type/impact/effort/site frontmatter and a Check: line, so triage can sort and delete items individually. All 11 Existing entries are carried over unchanged in substance.

The README also gains a ## Repo notes section with this repo's repro commands, guard-test conventions, and pre-ship checks, which triage reads on every item.
