Create permissible value (PV) lists for HTAN Phase 2 biospecimen data model

1. Create local and github /config, /cache and /permissible_values directories

2. Prepare a list of caDSR CDE ids, and put them into local and github /config/ca_dsr.txt

3. Run Ethan's code pull.py locally

4. Transfer files to relevant github project directories
