feat(openclaw): add heartbeat cost-control policy and legacy file repair

[fisherdaddy]

Empty/missing HEARTBEAT.md still triggers periodic model calls, and the bundled AGENTS.md template used to tell agents to invent routine checks. Add a managed heartbeat policy prompt, strip the proactive-heartbeat guidance from bundled templates, and repair legacy HEARTBEAT.md files in existing workspaces on sync.
