This PR

makes the naming of all the Holohub applications consistent across metadata.json ("name" field) and README.md (title),
improve the naming of some applications to be more descriptive,
remove redundant words from application names, such as "application", "holoscan", "holohub", etc.
fix README titles that does not starts with #.
