Replaces the huge teleport script with a much smaller one that's easier to maintain, also fixes an accidental regression in #1516 where you would walk forward after teleporting.
This currently doesn't support the special messages the old one has for adding ignored entities or disabling the teleporter entirely. If someone needs them, I can add them back in. This also doesn't support left-handed mode, but that can also be easily added if need be.
