Release commit for 1.42

        - stop using Module::Install for packaging, improving metadata and
          fixing Makefile.PL on perl 5.26+.  Generating a Catalyst application
          still uses Module::Install::Catalyst for now.
        - fix generated_app.t test on Windows
        - whitespace and formatting tweaks
        - various internal test cleanups
