This PR updates the issue template embedded into the notification prompt to create a new issue.
This was outdated compared to even our old issue template, this syncs it with the updated changes I've made in pulsar-edit/.github#342.
Ideally the org level update to the issues template should be merged first to make sure we are happy for this to follow it.
It is of course recreated in Markdown as I don't think you can do this with a YAML form...
