This pull request standardizes license headers across several configuration files, including .gitattributes, .gitignore, .readthedocs.yaml, and docs/.gitignore. It also cleans up comments in .yamllint.yaml and removes a duplicate license header block in qualtran/conftest.py. There are no review comments, so I have no feedback to provide.

In preparation for adding a license check step to the CI workflow, this adds the standard copyright & license comments to the tops of a few files that lacked them.
