fix(reviewer-bot): correct reviewer response status labels

Summary

• fall back to live current-reviewer PR reviews during status-label projection so legacy entries like [Coding Guideline]: Do Not Depend on Function Pointer Identity #256 do not incorrectly show status: awaiting reviewer response
• share current-reviewer review repair helpers across projection, reconcile, sweeper, and scheduled maintenance while keeping reviewer freshness timestamps monotonic
• add regression tests for the [Coding Guideline]: Do Not Depend on Function Pointer Identity #256-style missing-channel-state case, stale-head safeguards, contributor-newer handling, and scheduled reviewer-review repair
