As per https://github.blog/changelog/2026-03-19-hierarchy-view-in-github-projects-is-now-generally-available/ , blank issues are now always enabled for people with at least triage permission. Hence we don't need a special issue template for this.
As a bonus, I've added the milestone issue template which you could use for creating new milestone issues.
