Adds changelog for v2.1, links in README and HTML

- updated the `changelog.py` script for 2.1
- created changelog for 2.1 in `2.1/changelog.html`
- added links to changelog in README.md (root and 2.1)
- added links to changelog in HTML docs (updated macro & regenerated)
