Original choices leant too heavily on making things into lists. Whilst
some items need to be listified to maintain Markdown semantics whilst
copying the original PDF formatting as closely as possible:

 - **Armor Class** 14 (natural armor), 11 while prone
 - **Hit Points** 39 (6d10 + 6)
 - **Speed** 30 ft., burrow 10 ft.

...having actions etc also listified was a personal choice, and not a
faithful recreation of the original content. This gets the 5.1 Markdown
closer to the original intent again.

However, it does also reformat the ability tables in monster statblocks
to be score/mod/save. This is a deliberate choice to maintain
consistency between SRD formats. No values have been changed.

- apply patches automatically when making vaults
- make the mechanism of patching more obvious
- drive more things from the directory name, rather than hardcoding
- update the README to more accurately reflect how processing is done
(I blame macOS case insensitive filesystems, not my own carelessness)
