FFE forms step 2 - Add missing forms

This PR offers some of the missing features of #1644:

• Form plainte/suspicion de tricherie
• Form Attestation de stage AFC
• Form Attestation de stage AFO
• Limit players lists for T3/T4

TODO for next PRs:

• Add contact information and FFE licence (plugin data) for accounts, used to generate FFE reports (@Molrn ?)
• Replace multi-lines inputs by textareas (@timothyarmes ?)
• Pass the writer as a new account option for almost all the forms (@pascalaubry)
• Pass the arbiter as a new account option for the certificate forms (@pascalaubry)
• Add the possibility of marking the accounts with the ORGANISER RoleType (as we already do with CHIEF_ARBITER and DEPUTY_CHIEF_ARBITER)
