docs: add integration details and access steps

Adds a concrete "Integration Details" section to the README with touchpoints (how to work with @CharlieHelps in PRs/issues) and clear access steps for collaborators. Also clarifies setup (app install, least privilege) and permissions, and adds a short privacy note.

Changes

• Standardize headings and capitalization; keep scope docs-only.
• Add Owners, Touchpoints, Request Access, Setup, and Privacy sections.
• Include least-privilege install guidance and correct review-request permissions for org vs. personal repos.
