this pr should remove feedback modal and move the function to contact page
and make some adjustment to the feedback form fields

Based on the discussion in the support meeting, the suggested format for the new contact form:

Email
VenueID - searchable dropdown, similar to the profile page
Role - "I am a ... " options: Program Chair, Senior Area Chair, Area Chair, Reviewer, Author
Issues - Subset issue by topic to be more clear
Profile issues - Email change, Name change, profile activation, add institution, vouch, other
PC Issues - Assigning reviewers, problem with readers, customizing forms, other
Author/ Reviewer issues - problems reading assignment, problem with submission, change submission information, expertise question, other

I added some common categories for PCs and Authors/Reviewers, let me know what you think @racheljsmart @melisabok and let me know if I'm forgetting anything

I'm not sure about this. even excluding year it's already a super long list i think people will select the first options if it's a required field

i think the role names are not fixed.

info email is also used in terms and conditions and privacy policy
for user to report bugs and for user to voluntarily provide information

I'm not sure whether those should be changed too

Do you have a recommendation for how to better track which venues are emailing us for help? When PCs have multiple affiliations, etc, it becomes difficult to see who is contacting us for which venue.

What do you mean by this? We can include an other option as a catch all.

i think the current input for venue id is better, we can add a validation to make sure user fill in the venue id

What do you mean by this? We can include an other option as a catch all.

for example area chair may be used to refer to senior area chair in some conferences
