[release-1.5] chore(docs): document auth via dynamic plugins #2467

This is an automated cherry-pick of #2446

This change adds the configuration updates and examples related to
installing and configuring authentication providers from dynamic
plugins.  This change also fixes a couple small markdown lint warnings.
